Working with professionals
Phoenix Support works with people who use services, families, representatives, advocates, commissioners and health and social care professionals to provide safe, person-centred support.
We aim to communicate clearly, respond honestly and work in partnership around the person's assessed needs, agreed outcomes, rights and choices.
How we maintain quality
We review support with the person and, where appropriate and agreed, with their representatives and professionals. We use feedback, audits, supervision, incident reviews and complaints learning to identify improvements.
Where a person's needs or risks change, we review the support plan, involve the relevant people and take action to keep support safe and appropriate.
Our person-centred approach
No decision about me, without me.
The person is at the centre of planning, review and day-to-day support. We consider their wishes, goals, strengths, communication needs, culture, identity, relationships, privacy and rights.
We involve others with the person's agreement or where there is another lawful basis to do so.

Who we work with
Depending on the person's needs, we may work with:
- Kent County Council and other commissioners;
- care managers and social workers;
- NHS services, GPs, community teams and pharmacies;
- families, representatives and advocates;
- housing providers and landlords;
- other organisations involved in the person's care and support.
Our services
Phoenix Support is registered with the Care Quality Commission to provide the regulated activity of Personal care. Our services may include:
- homecare and domiciliary support;
- support for people living in supported living arrangements;
- community and outreach support;
- support with personal care, daily living, medication, appointments, independence and community participation, where included in the agreed plan.
How referrals are considered
We consider each referral individually. Before accepting a service, we review:
- the person's assessed needs, desired outcomes and communication needs;
- risks and the steps needed to provide support safely;
- whether the requested support falls within our service and registration;
- location, travel and the availability of suitable staff;
- funding, hours, proposed start date and commissioning arrangements;
- how we will work with the person and others involved in their support.
We will not accept a referral unless we believe we can meet the person's needs safely and well. If we need more information, we will explain what is required before making a decision.
Quality and assurance
Quality oversight is led through Phoenix Support's management and governance arrangements, including the Registered Manager, Nominated Individual and Directors.
Our assurance arrangements include:
- safer recruitment, appropriate checks and employment references;
- induction, training, competency checks, supervision and support for staff;
- person-centred support plans and risk assessments;
- medication governance and competency monitoring where medication support is provided;
- care records, audits, reviews, spot checks and management oversight;
- safeguarding, incident reporting and Duty of Candour processes;
- complaints, compliments, comments and feedback;
- learning from incidents, concerns, audits and service reviews;
- data protection, confidentiality and secure record keeping.
