Learning Disability Support

Phoenix Support provides person-centred support for adults with learning disabilities. We support people in their own homes, in supported living and in the community.

We listen to what matters to you. We work with you to build support around your needs, goals, choices and communication preferences.

What the service is

Our support is designed to help you live as safely, independently and confidently as possible. The amount and type of support will depend on your assessed needs, agreed outcomes and funding.

We support adults aged 18 and over, mainly in Maidstone, West Kent and surrounding areas of Kent. Whether we can provide a service will also depend on safe staffing, travel and whether we can meet your needs well.

Your choices

No decision about me, without me.

You should be involved in decisions about your support. We will ask what is important to you, how you prefer to communicate, what you want to achieve and how you want support to be provided.

With your agreement, we can involve family members, advocates, representatives and professionals. We will respect your privacy, dignity, rights, culture, identity and personal preferences.

Depending on your support plan, we may help with:

Personal care and daily routines
Medication support
Shopping, cooking and preparing meals
Household tasks and looking after your home
Attending health appointments and following agreed health plans
Communication, making choices and understanding information
Building daily living skills, confidence and independence
Accessing the community, activities, education or work opportunities
Maintaining relationships and staying connected with people who matter to you
Staying safe and managing agreed risks

We will be clear about what we can provide. If another service or professional is needed, we will discuss this with you and the people involved in your support.

How support starts

Contact us, or ask your social worker, care manager, family member, advocate or another professional to contact us.
We talk with you about what support you need and what matters to you.
We complete an assessment to check whether we can meet your needs safely and well.
If the service is suitable and funding is agreed, we develop a person-centred support plan with you.
We plan the start of your support, introduce the team and review how things are going.
